Martha Baker, LPC

Martha is a Licensed Professional Counselor (LPC) in the state of Georgia. She received her Bachelor of Science degree in Psychology from Bridgewater State University in Massachusetts in 1988 and her Master of Arts in Agency Counseling from the University of Colorado in 1997. She holds a certification for Eye Movement Desensitization and Reprocessing (EMDR) and she is certified in CBCT.

Martha has worked providing counseling to individuals of all ages, couples and families in private practice, therapeutic foster care, community health care, and an outpatient opiate addictions clinic. She is skilled in many techniques to engage in building a safe and compassionate environment in order to assist those who take the difficult step to enter into the therapeutic relationship.

Martha is in network with Aetna, Ambetter, BCBS, Amerigroup, Cigna, Humana/tricare, Magellan and Meritain.

After scheduling by calling 678-666-4717 Ext 101 or emailing [email protected], and prior to our first meeting, please complete the appropriate intake packet found below.  On this page, there is a link to an Adult Intake Forms,  and Couple’s Intake Forms.   Once you fill out and sign your forms through Docusign, they will go to my email for my signature’s and I will then upload them to your Theraportal account.  In the case of a couple, the initial person will fill out the form, it will be emailed to the second member of the couple to have them fill their part out, then it will go to the clinician for signing and uploading.

Please do not fill out paperwork prior to scheduling an appt with the office by calling 678-666-4717 ext 101.

The below link will bring you to the video counseling platform. This is a HIPPA compliant site. I will assure privacy at my end. You might consider finding a private space at your end to ensure full privacy. It is important that you do not try to click on telehealth in your theraportal account. That does not work as the doxy link is separate from your theraportal and our practice software.
