Hello and welcome! I’m so glad you’ve chosen to start this journey with me, and I look forward to meeting you at our first appointment. To make sure we’re ready to dive in, there are just a few steps to complete before we can begin.

Before your session, please complete and submit the appropriate DocuSign forms listed below. These need to be filled out with your full legal name and signature, and uploaded into your patient portal. Please note that I’m unable to meet with you—whether in-person or via telehealth—until all paperwork has been completed and uploaded. If you run into any issues, our help desk is here to support you at 678-666-4717 ext. 101.

You’ll receive login information for your patient portal via email. If you haven’t received it, feel free to reach out to the help desk at 678-666-4717 ext. 101, and we’ll make sure you’re set up.

Appointment Management:

  • Need to cancel or reschedule? You can do so through your patient portal or by calling the main office.
  • Please note that appointments canceled less than 24 hours in advance will incur a missed appointment fee as outlined in your paperwork.
  • If you encounter any issues connecting for your appointment, don’t hesitate to call or text me directly at 678-591-0345, and I’ll help you troubleshoot.

If you arrive more than 10 minutes late to an in-person or telehealth session, it may be considered a late cancellation and subject to the missed appointment fee outlined in your paperwork.

For Parents: If your child or adolescent is the client, please be sure to attend the first session and remain present throughout.

Meetings via Telehealth: All individuals in the session must be together in the same location to use the connection. My HIPAA compliant telehealth software does not allow for more than one client location.
