Hello!

I’m looking forward to meeting you for our first appointment. To help ensure that we’re ready to begin, please review the following important information and complete all required paperwork before our session.

Before beginning treatment, all intake forms must be completed and submitted electronically. You can access the forms through the links below and complete them via DocuSign. Once finished, I will receive the paperwork, complete my section, and you will then upload the finalized forms to your TheraPortal account.

Important for new Clients

As part of your psychiatric evaluation and ongoing treatment, please list all current medications you are taking at home, including:

  • Prescription medications
  • Over-the-counter medications
  • Vitamins and herbal supplements

Accurate and up-to-date medication information helps ensure your treatment plan is safe and effective. Please include this information in your intake paperwork or bring a current medication list to your first appointment.
